What to expect as a New Patient
For many, coming to a new chiropractic office can be intimidating, especially if this is your first time seeing a chiropractor. At Franklin Chiropractic, we start with a 10-minute complementary phone consultation to first determine if we can help with your health concerns.
Once you have made your New Patient appointment, you will receive an email confirmation for your first visit. Next, please fill out our online New Patient Intake Form, which will be emailed to you via the Jane App, our Electronic Health Record (EHR). This intake needs to be completed 2 hours prior to your scheduled time with the doctor. This intake form gives Dr. Franklin consent to treat. Your detailed health concerns and health history will help Dr. Franklin prepare for your visit and provide him with the knowledge he needs to help you reach your health goals. We reserve the right to cancel your appointment if the intake is not completed in time.
